Green’s Neuropsychology and Consultation Services (GNCS) uses your information only to directly contact you; we never sell, distribute, or use your information to contact you for promotional offers. To protect your privacy over the internet, we ask that you provide only the necessary information to have effective communications and limit providing private information that you want protected from internet attacks.
The only information we collect and maintain is that information you have provided for clinical, forensic, business consultation, or Federal Aviation Administration evaluations. We are required by law to maintain records for up to seven years. After that time, your information will be deleted from our system.
We maintain all private information on encrypted computers and encrypted files to minimize the possibility of compromise. Any information covered under the Privacy Act, 5 USC 552(a), and/or the Health Insurance Portability and Accountability Act (PL 104-191) and its various implementing regulations must be protected in accordance with those provisions. We ensure these provisions are upheld. Healthcare information is personal and sensitive and must be treated accordingly.
If correspondence from GNCS contains healthcare information, it is being provided to you after appropriate authorization from the patient/client or under circumstances that don’t require patient authorization. You, the recipient, are obligated to maintain it in a safe, secure, and confidential manner. Redisclosure without additional patient consent or as permitted by law is prohibited. Unauthorized redisclosure or failure to maintain confidentiality subjects you to application of appropriate sanction. If you have received any correspondence in error, please notify the sender at once and destroy any copies you have made.
